Following the success of the 2024 sessions, Vermont South Neighbourhood House has invited Melanie to return in 2025 to lead this practical and compassionate workshop on end-of-life planning.

You'll learn how to:

  • Understand and complete your Advance Care Directive

  • Appoint a Medical Treatment Decision Maker and Support Person

  • Document your personal values and wishes using a Complimentary End of Life Wishes Planning Form
